The parties, for valuable consideration hereinafter state, on the date entered below, by and between Park Personal Defense Training, LLC, Kelly Park, and Jill Park, and any other instructors they employ and the facility and owners of the facility or location in which you receive instruction, hereinafter called the “Instructor” and the undersigned hereinafter called the “Participant” agree as follows: 1. In consideration for being allowed to participate in gun training, learn from the instructors and use the facilities they provide for training, Participant waives any claim arising from such gun training or any related use of the facilities used to host said gun training. 2. Participant, their heirs, personal representatives, agents, assignees, or anyone else claiming to have an interest in the claims of Participant also waives, releases, and forever discharges Instructor and their officers, agents, employees, and representatives from all responsibilities or liabilities from injuries or damages arriving out of or connected with their attendance in gun training, the instructions they receive or any use of the related facilities. 3. Participant acknowledges that the use of firearms in close proximity with other users learning how to safely use a firearm involves a risk of injury or could be dangerous no matter how the instruction is organized or how the facility is designed or maintained since the use of firearms is an inherently dangerous activity. Knowing all this, Participant hereby expressly assumes and accepts any and all risks of injury or death that occur on or result from being within the facility used to complete said gun training. 4. The parties agree that this form may be signed once per year and shall be kept on file for whenever Participant may attend gun training. Any refunds for class will be given minus the credit card processing fees.*
